Failure to use information made available to him. I did everything to the letter he ask me to but to no avail. In the end none of my "cleanliness" (efforts to do the right thing) counted. Mr Smith was silent in court and never stood up for my rights or my childrens well being. Having paid out an enormous sum of money, I feel cheated.
